NAICS code for a dental office: 621210, and the codes around it

Every kind of dentist, from a solo family practice to an oral surgeon, shares one NAICS code. The decisions are about the businesses around the dentist: the management company, the lab and the hygienist each have a code of their own.

Industry code6 min readPublished

The NAICS code for a dental office is 621210, Offices of Dentists: practices of DMDs and DDSs offering general or specialized dentistry, including orthodontists, endodontists, periodontists, prosthodontists and oral surgeons. The SIC code is 8021, Offices and Clinics of Dentists. A dental lab is 339116, an independent hygienist's office 621399, and a company that manages dental offices for others (a DSO's management arm) fits 561110, Office Administrative Services.

  • General dentists and every dental specialty share 621210; the specialty shows up in the NPI taxonomy code instead.
  • Dental labs (339116) are manufacturing, and independent hygienists (621399) are other health practitioners.
  • The Census index lists managing offices of dentists under 561110, which is where a DSO's management company fits.
  • County Business Patterns counted 135,665 dental offices with paid employees in 2023; 73.1% had fewer than ten.
  • The SBA's size standard for 621210 is $9.0 million in average annual receipts.
On this page

This page is for dental practices, group practices and dental service organizations (DSOs) filling in a form that asks for an industry code (a practice loan, insurance, a vendor or government registration, a tax return), and for companies that sell to dentists and need to filter by code. For the market itself, how practices buy and the rules for emailing them, see the guide to dental practices.

The NAICS codes for dental businesses

NAICS 2022 codeOfficial titleWhat it covers
621210Offices of DentistsPractices of D.M.D., D.D.S. or D.D.Sc. dentists in general or specialized dentistry or dental surgery, in their own offices or in hospitals and HMO centers. Includes endodontists, orthodontists, periodontists, prosthodontists, oral pathologists and oral and maxillofacial surgeons.
339116Dental LaboratoriesMaking dentures, crowns, bridges, artificial teeth and orthodontic appliances to order for dentists.
621399Offices of All Other Miscellaneous Health PractitionersIncludes dental hygienists' own offices and denturists who take impressions and fit dentures.
561110Office Administrative ServicesDay-to-day administrative services for others on a contract or fee basis; the Census index lists "managing offices of dentists" here.
621498All Other Outpatient Care CentersCommunity health clinics with practitioners from several fields, including dentists.

How to choose

For a practice, it's simple: if licensed dentists provide the care, the code is 621210, whatever the specialty, the number of locations or the payer mix. The Census definition covers "private or group practices in their own offices (e.g., centers, clinics) or in the facilities of others." The hard cases are the businesses that sit next to the practice:

If the business mainly...NAICSSIC
Provides dental care by licensed dentists, general or specialist6212108021
Is a mobile or school-based dental practice run by dentists6212108021
Makes crowns, dentures, aligners or appliances to a dentist's order3391168072
Is a hygienist's own office (where state law allows)6213998049
Provides billing, HR, marketing and management to dental practices (a DSO's management company)5611108741
Owns and manages its own dental practices as a corporate head office551114, Corporate, Subsidiary, and Regional Managing OfficesNo direct SIC equivalent
Is a community health center with dental, medical and behavioral care6214988093
Sells dental equipment and supplies423450, Medical, Dental, and Hospital Equipment and Supplies Merchant Wholesalers5047
Manufactures dental equipment and supplies339114, Dental Equipment and Supplies Manufacturing3843
Trains dental hygienists611519, Other Technical and Trade Schools8249

In a typical DSO structure, the dental practices stay 621210 and the management company that serves them is 561110. If one company both owns the practices and runs them centrally, its head office is 551114 and each practice is still 621210. The practice's code doesn't change when it signs with a DSO.

SIC code for dental offices

The SIC code for a dental office is 8021, Offices and Clinics of Dentists, and the Census concordance maps it to 621210 one to one. Dental laboratories are SIC 8072, which maps to 339116. That makes dentistry one of the easier industries to convert in either direction. The SEC's own SIC list has no 8021; the only dental companies on EDGAR are suppliers, under codes such as 5047 and 3843.

NPI taxonomy codes: the codes insurers use

Dental claims and the CMS NPI registry don't use NAICS. Each dentist and practice that bills insurers has a National Provider Identifier with a Health Care Provider Taxonomy code from the National Uniform Claim Committee. That's where the specialty is recorded:

Taxonomy code (NUCC 26.1)Specialty
122300000XDentist
1223G0001XGeneral Practice
1223X0400XOrthodontics and Dentofacial Orthopedics
1223P0221XPediatric Dentistry
1223S0112XOral and Maxillofacial Surgery
1223E0200XEndodontics
1223P0300XPeriodontics
1223P0700XProsthodontics

So if you need orthodontists only, filter the NPI registry by 1223X0400X rather than looking for a NAICS code that doesn't exist.

How many dental offices there are

County Business Patterns (CBP) 2023, establishments with paid employees:

NAICS codeEstablishmentsEmployeesAnnual payroll
621210 Offices of dentists135,6651,028,889$62.5 billion
339116 Dental laboratories4,78242,188$2.3 billion
Employees per office621210 Offices of dentistsShare

10 to 1929,79022.0%
20 to 496,1134.5%
50 or more5440.4%

California has by far the most dental offices (22,757), followed by Texas (11,902), Florida (9,039), New York (8,001) and Illinois (5,781). The average office has about 7.6 employees and pays about $60,700 per employee (our calculation).

CBP counts locations with paid employees, so a 40-office DSO-affiliated group counts 40 times, and it leaves out practices with no employees. It uses NAICS 2017; 621210 didn't change in 2022.

SBA size standards

From the SBA's Table of Size Standards, effective March 17, 2023 (13 CFR 121.201 on the eCFR shows no later change):

NAICS codeSize standard
621210 Offices of dentists$9.0 million in average annual receipts
621399 Other health practitioners (including hygienists)$10.0 million
561110 Office administrative services$12.5 million
339116 Dental laboratories500 employees
423450 Dental and medical equipment wholesalers200 employees

Receipts are averaged over the latest five fiscal years and include affiliates. For practices tied to a DSO, whether they count as affiliates depends on control under the SBA's affiliation rules, so check before you certify as small.

Where the code is used

  • Practice loans. SBA lenders check 621210 against the $9.0 million standard, including affiliates.
  • Tax returns. The IRS activity code is the same: 621210, Offices of dentists. A dental lab's sole owner uses 339110, Medical equipment and supplies mfg., and a management company 561110.
  • Federal work. If you bid for federal dental contracts, register 621210 as your primary code in SAM.gov; the contracting officer sets one code per contract.
  • Insurance. Tell your insurer about any lab or management company the group adds, since each is a different kind of business.

Finding dental offices by code

There are no listed dental practices on EDGAR. The public companies in dentistry are suppliers, which file under supply and manufacturing codes:

CompanyEDGAR SICHeadquartersRevenue, 2025 10-K
Henry Schein5047 Wholesale-Medical, Dental & Hospital EquipmentMelville, NY$13.18 billion
Align Technology3842 Orthopedic, Prosthetic & Surgical AppliancesTempe, AZ$4.03 billion
Dentsply Sirona3843 Dental Equipment & SuppliesCharlotte, NC$3.68 billion

For practices, the NPI registry is the best free source: search organizations by taxonomy and state to get practice names, addresses and phone numbers, or download the full NPPES file. A Bebop search can describe the practices you want (multi-location orthodontic practices in Arizona with an office manager, say) and return the practices and decision makers with the reason each fits.

Worked example: coding a DSO-affiliated group

Illustrative example. The companies and figures are invented.

Brightline Dental Partners (brightline.example) is a management company serving 14 dental practices in Georgia. Each practice is owned by its dentists. Brightline also runs a small crown and bridge lab that serves the practices.

  1. The practices. Each is 621210 (SIC 8021), with NPI taxonomy 1223G0001X for general practices and 1223X0400X for its two orthodontic offices.
  2. Brightline. Its main business is billing, HR and marketing for the practices under contract, so it's 561110 (SIC 8741). The lab is a second code, 339116.
  3. Size. A practice with $2.4 million in average receipts is far under $9.0 million on its own. Whether Brightline's revenue counts as an affiliate's depends on control, which is a question for counsel.
  4. Lists. A seller of imaging equipment targets the 14 practices by NPI; a seller of payroll software targets Brightline, where the decision sits.

Next step: read the guide to dental practices for who decides in a practice and how to build a compliant list.

Questions people ask

What is the NAICS code for a dental office?

621210, Offices of Dentists. It covers the practices of licensed dentists (D.M.D., D.D.S. or D.D.Sc.) in general or specialized dentistry or dental surgery, in their own offices or in hospitals and other facilities. Solo practices, group practices and multi-location practices all use it. The SBA's size standard is $9.0 million in average annual receipts, and the SIC equivalent is 8021.

What NAICS code do orthodontists and oral surgeons use?

The same as general dentists: 621210, Offices of Dentists. The Census index lists orthodontists', endodontists', periodontists', prosthodontists' and oral and maxillofacial surgeons' offices there. NAICS doesn't separate dental specialties. To find or label a specialty, use the NPI taxonomy code instead, such as 1223X0400X for orthodontics or 1223S0112X for oral and maxillofacial surgery.

What is the SIC code for a dental office?

SIC 8021, Offices and Clinics of Dentists. The Census concordance maps it one to one to NAICS 621210, so converting between the two is straightforward. Dental laboratories are SIC 8072 (NAICS 339116). The SEC's SIC list doesn't include 8021, which is why no dental practices appear as a category on EDGAR.

What NAICS code should a dental service organization use?

A DSO's management company, which provides billing, HR, marketing and other business services to practices under contract, fits 561110, Office Administrative Services; the Census index lists managing offices of dentists there. The practices themselves stay 621210. If one company owns and centrally runs its practices, its head office fits 551114, Corporate, Subsidiary, and Regional Managing Offices.

What is the NAICS code for a dental lab?

339116, Dental Laboratories, in manufacturing: making dentures, crowns, bridges, artificial teeth and orthodontic appliances to order for dentists. County Business Patterns counted 4,782 dental labs with paid employees in 2023. Because it's a manufacturing code, the SBA size standard is 500 employees rather than a receipts limit. The SIC equivalent is 8072, Dental Laboratories.
