NAICS code for a med spa: 621111 or 812199, decided by what the treatments are

This page is for med spa owners filling in a form that asks for a NAICS or SIC code (a loan, an insurer, a merchant account, a tax return) and for sellers building lists of aesthetics businesses.

Why there's no med spa code

The Census Bureau's NAICS index names most of what a med spa does, but in different places. "Day spas" and "hair removal (i.e., depilatory, electrolysis, laser, waxing) services" are in 812199. "Esthetician (i.e., skin care) services" and "facial salons" are in 812112. "Dermatologists' offices" and "plastic surgeons' offices" are in 621111. "Medical spa" isn't there at all. So a med spa takes the code for whichever side earns more.

The NAICS codes a med spa can use

How to choose: medical or personal care

A spa inside a resort follows the hotel (721110), and a fitness spa without rooms is 713940. If you sell several of these, the one with the most revenue sets the primary code: the IRS asks for "the principal source of your sales or receipts."

The two-entity structure

Med spas are often split in two: a practice owned by a physician or nurse practitioner that delivers treatments, and a management company that typically holds the lease, equipment and brand and provides non-clinical services under a management services agreement. AirSculpt Technologies, a body contouring company, describes this setup in its 10-K: because of state limits on the corporate practice of medicine, its centers operate through physician-owned entities that handle clinical work, and its subsidiaries provide non-clinical support under management services agreements.

In that structure each entity gets its own code. The practice is 621111 or 621399. The management company is 561110, Office Administrative Services, where the Census index lists "managing offices of physicians and surgeons." Check your state medical board's rules before you choose the structure.

SIC codes for med spas

SIC 7299 is a catch-all that also maps to 541990, 561311 and 812990, so a SIC 7299 list is mostly not spas. On SEC EDGAR, AirSculpt files under 8011. To convert other codes, use the NAICS to SIC converter.

How many med spas there are

No federal source counts med spas, because there's no code to count them under. County Business Patterns (CBP) 2023 counts the codes they share, for establishments with paid employees:

In 812199, 20,883 locations (65.5%) have fewer than five employees and 161 have 50 or more. The top states are California (3,882), Florida (2,869), New York (2,656), Texas (2,637) and Illinois (1,135). Med spas coded 621111 are hidden inside the 204,617 physician offices. CBP 2023 uses NAICS 2017; none of these codes changed in 2022.

Where the code is used and what a wrong one costs

• Insurance. Medical malpractice and salon liability policies cover different risks. Give your insurer the code that matches the treatments you perform.
• Tax returns. The IRS Schedule C list has 621111 for physicians, 621399 for other practitioners, 812112 for beauty salons and 812190 for other personal care services (including diet and weight reducing centers).
• Lists. A med spa coded 812199 sits with tattoo parlors and waxing salons, and one coded 621111 with cardiologists. Neither code alone finds med spas.

Finding med spas

EDGAR has almost nothing: AirSculpt, filed under SIC 8011 and headquartered in Tampa, reported $151.8 million in 2025 revenue. Most med spas are private, so the public sources are state licensing records (medical boards for supervising physicians and nurse practitioners, cosmetology boards for estheticians) and the treatments a spa lists on its own site. A Bebop search takes plain-language criteria (offers injectables and laser treatments, number of locations, medical director on staff, state) and returns med spas and contacts with the reason each fits. Worked example: coding a med spa

Illustrative example. The company and figures are invented.

Solstice Aesthetics (solstice.example) in Arizona runs two locations through a nurse practitioner's professional entity and a management company that the same owners control. Last year: injectables $1.6 million, laser resurfacing and medical-grade peels $0.7 million, laser hair removal $0.5 million, facials $0.4 million, skin care products $0.3 million. Average annual receipts over five fiscal years, both entities together: $2.9 million.

1. Primary code. Medical treatments are 66% of revenue ($2.3 million of $3.5 million), delivered by the nurse practitioner's practice: 621399 (621111 if a physician owned it).
2. Extra code. 812199 for laser hair removal and spa services.
3. Management company. 561110 and SIC 8741.
4. Size check. Common control makes the entities affiliates; $2.9 million is under every standard above.
